COVID Alert Form: If you have tested positive for COVID, you and your household must complete the COVID Alert Form

Household isolation: according to government guidelines, double vaccinated people (>14 days since second dose vaccinated in the UK) will no longer be legally required to self-isolate if they are identified as a close contact of a positive COVID case.

However, your household would still have to complete the COVID Alert Form to submit evidence of vaccination status for proof of your exemption.

If you were vaccinated abroad, confirming your vaccination status (and providing evidence) with your GP for proof on your NHS records may mean you can be exempt from self-isolation if you are a close contact.
